One of the OAA’s (Older Americans Act) foundational principles is that the programs and services created to help support consumers in their homes and communities are customized to meet their individual needs. There’s nothing one-size-fits-all about AAAs or the services they offer their clients!

Roles of all AAAs: **Assess community needs and develop and fund programs** that respond to those needs; **educate and provide direct assistance** to consumers about available community resources for long-term services and supports; **serve as portals to care** by assessing multiple service needs, determining eligibility, authorizing or purchasing services and monitoring the appropriateness and cost effectiveness of services; and **demonstrate responsible fiscal stewardship** by maximizing use of public and private funding to serve as many consumers as possible.

#### Program Eligibility

Meals-on-Wheels helps seniors remain in their own homes by providing hot lunchtime meals for those age 60+. We provide a safety check for our seniors, giving peace-of-mind to family members who must work during the day. If in the unlikely event a senior needs immediate medical assistance, drivers and volunteers will call 911 and remain with the senior until help arrives.

**Service Days:**

Meals are delivered Monday through Thursday, excluding holidays. The number of service delivery days is dependent upon need.

Special diets and (limited) liquid meals are available for those at higher nutritional risk.

**Eligibility:**

60 years of age or older and homebound, and the caregiver spouse of the homebound senior, and the disabled child of a homebound senior. Homebound is someone that is unable to leave their home without assistance. Those individuals who still drive and get out into the community are not considered homebound. Meals-on-Wheels encourages those that are not homebound to participate in lunch at their senior center.

**Fees & Charges:**

*   Fees are not charged for this program.
*   A contribution of $2.50 per meal is a suggested donation for both congregate and home delivered meals.

Six County Office Hours – Monday-Friday 8:00 a.m. – 5:00 p.m.

Prior to beginning Meals-on-Wheels, a staff member will visit the senior’s home to assess their needs and make a final determination for eligibility. A waiting period for services may apply due to insufficient funding or delivery capacity.

Service levels will depend on the ability of the older individual to help themselves, and the capacities of family members and caregivers in and out of the home to provide assistance.

Some areas are considered under-served in that Seniors may qualify for the program but delivery may not be available. Alternative delivery methods may be considered. Some routs may be put on hold if total numbers fail to meet minimums.

Six County Association of Governments

For over 35 years, the Administration on Aging has provided home and community-based services to millions of older persons through the programs funded under the Older Americans Act. The Utah Division of Aging and Adult Services serves as a link between the national Administration on Aging and local programs, Area Agencies on Aging. Area Agencies administer a wide variety of home and community-based services for Utah residents who are 60 and older. The goal is to ensure services are provided statewide that allow people to remain independent.

  

Local Area Agencies on Aging have been designated to cover geographic regions of the state of Utah and are charged with the responsibility of providing a comprehensive array of services to, and advocating for, the needs of seniors residing in their areas.

  

 The Division administers the Adult Protective Services program to investigate cases of abuse, neglect, and exploitation of vulnerable adults. Trained staff in a statewide system of offices, work with local law enforcement and community partners to educate and assist victims to access appropriate resources within the community.

## Alternatives & Caregiver

**Alternatives-** The case manager assists frail, older individuals to remain in their home and not to be prematurely placed in a nursing home. The case manager completes a comprehensive assessment with the individual to determine eligibility, and together they develop a plan of care to meet the individual’s needs. Services include: home health aides, homemakers/personal care, emergency response systems and other services are coordinated to maximize longevity in the individuals own home. **Caregiver-** The Caregivers Support Program provides information, assistance, support groups, training for caregivers who are caring for a loved one 60 years or older who are suffering chronic long term illness or conditions where the intensity of such care giving creates stress and other informal relief is not sufficient. Active Re-Entry is a community based program which assists individuals with disabilities to achieve or maintain self-sufficient and productive lives in their own communities and home.

The mission of Tooele County Aging Services is to promote the safety, health, and independence of adults, 60 years of age and older by supporting a coordinated system of services that will include:

Tooele County Aging Services Department serves as the organization unit with delegated authority for implementing the Older Americans Act with Tooele County. The Division of Aging Services serves as an effective and visible advocate and the leader relative to all Aging issues for seniors in the County. The Division pro-actively carries out a wide range of functions related to advocacy, planning, coordination, inter agency linkages, information sharing, brokering, monitoring and evaluation designed to lead to the development or enhancement of comprehensive and coordination community based systems serving the County.

## Meals on Wheels

**Meals for Homebound Seniors** Meals on Wheels nutritious midday meals are delivered to frail and isolated older adults Monday through Friday (weekend meals are delivered Thursday and Friday). Special diets (cut meat, mechanical soft, pureed, no nut, no pork and no seafood) are available. All meals are low-sodium, low-calorie and low-fat.

Each Meals on Wheels applicant is evaluated for program eligibility and additional services needed to assist in living as independently as possible.
